how do i get reimbursed after seeing an open access provider?

We are in-network for Medicare and most Medicare Advantage Plans, most Anthem, United Healthcare, Aetna, Humana and UMR plans. Call us or text us at 812-637-1300 with your information and we can chek your particular plan for participation.

What medical plans do you accept?

There are two types of insurance that may cover eye care, vision insurance and health (medical) insurance. The insurance that you'll use depends on what issues you are having. Vision insurance is for routine eye health screening and to receive a prescription for glasses and/or contact lenses. Medical insurance is billed for ALL non-routine visits including diabetic exams.

What is covered by insurance?

As an open access provider we are not required to use specific lens or frame brands and are not required to use insurance owned lens labs. This means a wider selection of frames and lens options for you, the patient. You also are eligible for our in office discounts, as well as reimbursement from your insurance company. This means in many cases you receive double the benefit amount! 

We are happy to help you complete any reimbursement forms for your insurance plan.

what is an open access provider?

We are currently in-network for Eyemed, inlcuding Humana Vision and Aetna Vision, and VSP. We are open access providers for the majority of other vision plans.
